Abstract

The present study tried to identify health problems , self care behaviors and needs for
professional referral. They ’ll be provided with database of continuity care system for
cancer patients.
Samples were selected from 97 patients hospitalized secondly or the more during the
period of Sept ember-December, 1998.
Authors adopt ed the modified version of McCorkle & Young’s Symptom Distress Scale
and Suh’s Self Care Checklist for the study.
The results showed that most respondents had tireness and unhappiness. Besides 70% of subjects had appearance change, pain , anorexia , disturbance of mobility, bowel elimination problem and need of somebody for consulting. Among these problems anorexia, nausea/vomiting and elimination problem were perceived very severiously . Total mean score ofseverity of health problems was 44.18. The worse perception of health, the more severity of health problem. Selfcare behaviors done by patients themselves were activities related to diet positive attitude, the balance between activity and rest and taking a sleep. Mean number of self care behavior was 11.18. There were more various self care behaviors in middle and high schooler than primary schooler and in the group having discomfort within three days and between from 21 days to 28 days than in the group between 3 days and 21days . As the needs of being some help from medical professionals , the subjects wanted them to be involved at the diet , pain management and guide to activity of daily living.
At the above results home patients after discharge needed to get a continued care
because of having various health problems . Health professional has to be involved in the
continuing care for the mor e effective patient management . Also the study r ecommends
that nur se who are in charging of discharge or work in the community agency wer e more
available than home care nurse for care continuity.
